For hospitals & discharge planners
Plan the next stepafter inpatient care.
For patients who need continued structured mental health treatment and can safely receive care outside the hospital, contact Psycamore to discuss PHP or IOP screening.
Coordinate a referral
Start with a call to the receiving office.
Contact the team before finalizing the treatment plan. Program fit, screening availability, and admission arrangements need to be confirmed for each patient.
01
Discuss the patient's needs
Share the reason for referral, age group, anticipated discharge date, and preferred location. Ask which program may be appropriate and when screening is available.
02
Coordinate the handoff
Confirm with the office which records are needed and how to send them. Arrange the transfer of relevant clinical information and confirm any required permissions.
03
Confirm the next appointment
Clarify screening details, transportation or virtual access, insurance verification, and the patient's follow-up plan. Confirm who to contact if arrangements change.
Before you call
Have the essentials ready.
- Patient and guardian contact details, when applicable
- Reason for referral and current treatment needs
- Expected discharge date and referring clinician's contact
- Current medications and relevant discharge information
- Insurance details for benefit verification
- Preferred location, schedule, and transportation needs
Call the office to coordinate clinical records directly. The website's screening request form starts a conversation; it is not the place to upload discharge records.
